The human eye is shielded by the conjunctiva, a thin, moist, and transparent membrane that covers the inside of the eyelids and the sclera (the white part of the eyeball). This outermost protective coating is rich in nerves and tiny, fragile blood vessels. Usually barely visible, these vessels can become engorged and prominent during inflammation.

Because their walls are delicate, these vessels can break easily. When a rupture occurs, a drop of blood becomes trapped between the sclera and the clear conjunctiva, resulting in a subconjunctival haemorrhage. Visually, this appears as a sharply outlined, bright red or dark red patch over the white of the eye. In severe cases, the entire sclera may be covered by blood.

  • Natrum muriaticum: Indicated for muscular asthenopia and blepharitis with lachrymation and scalding. A hallmark symptom is ciliary neuralgia that comes and goes with the sun, alongside letters running together while reading.

  • Gelsemium: Acts primarily by paralyzing the third nerve, which supplies the circular fibers of the iris. This paralysis dilates the pupil, causes ptosis (drooping eyelids), strabismus, and diplopia (double vision).

  • Belladonna: Stimulates the sympathetic radiating fibers of the iris to overcome the circular fibers, leading to dilated pupils. Presents with sudden, violent pain, red conjunctiva, and eyes that feel swollen, protruding, and full of sand. Features great photophobia; desire to rub the eyes brings relief.

  • Physostigma: Contrasts directly with Belladonna by stimulating the third nerve to contract the pupil. It will effectively contract a pupil dilated by Belladonna, but will not contract a pupil paralyzed and dilated by Gelsemium.

  • Spigelia: Indicated for ciliary neuralgia with radiating pains and a distinct cold feeling in the eye. (Thuja also features this specific cold sensation).

  • Cinnabaris: Characterized by a highly specific ciliary neuralgia: a pain that starts from one canthus (corner), traces around the brow, and ends at the other canthus.

  • Comocladia dentata: Severe pain in the right eye as if pushed out of the head, which is uniquely worse near a warm stove.

  • Paris quadrifolia: A distinct sensation as if the eyes were drawn back into the head by strings.

  • Causticum: Features paralysis of the eyelids, muscular weakness, double vision, and a burning, sandy sensation.

  • Euphrasia: Traumatic conjunctivitis and blepharitis with thick, excoriating tears that scald the cheeks, coupled with photophobia that is worse in artificial light.

    • Differential: In Euphrasia, the eye discharge excoriates while nasal discharge is bland. In Allium cepa, the exact opposite occurs: the nasal discharge excoriates the lip, while the eye discharge is bland.

  • Argentum nitricum: Indicated for violent, purulent ophthalmia. The core characteristic is the sheer profuseness of the thick, yellow, bland discharge.

  • Pulsatilla: Conjunctivitis featuring a thick, yellow, bland discharge. Specifically indicated for ophthalmia neonatorum or ophthalmia following measles.

  • Graphites: Inflammation localized strongly at the canthi. The edges of the eyelids tend to crack and bleed. Associated with inward-turning lashes, excoriating discharges, styes, and corneal vesicles.

  • Hepar sulphur: Purulent conditions, including hypopyon (pus in the anterior chamber). Symptoms are aggressively worse from cold air or cold applications.

  • Ferrum phosphoricum: Inflamed, red eyes with a sensation of grains of sand under the lids. Pain upon moving the eyes and photophobia exacerbated by artificial light.

  • Aconite: The first defense for conjunctivitis resulting from dry, cold winds or a foreign body. Features intense heat, burning, violent aching, and photophobia. If Aconite fails to resolve a foreign body irritation, Sulphur is the follow-up remedy.
